IMPORTANT NOTES:

• All camp balances and paperwork must be submitted no later than 2 weeks prior to camp’s start date.

 

Mandatory paperwork includes:

- Participant Release of Liability form

- Medical Information and Release form

- Physical Examination and Immunization Records
(must have been done within 12 months of camp’s start date per Massachusetts State Board of Health)

Paperwork can be mailed, emailed or faxed to RBI Academy.
